Birth of the twins (8.1.1) Storyboard: Birth (8.1)
Linda Lambert is 38 weeks pregnant with twins when she goes into labour. She presents to the hospital, is admitted and is taken to the labour and delivery ward.
The midwife who has followed Linda through the pregnancy assesses her and the presentation of each twin is favourable for a vaginal delivery. Throughout the labour, the obstetrical nurse keeps a record of the events. Eight hours later Linda gives birth to the first twin. Ten minutes after that the second twin is born. While the twins are being attended to by the pediatric medical and nursing staff, the unit clerk creates an entry for each in the hospital’s ADT system. The midwife creates her record of the delivery, which is included in the mother’s EMR. She also completes the provincial registration of birth form and submits it. The obstetrical nurse completes her record of the events, which is added to the mother’s and babies’ EMR. The pediatric physician creates a record of the newborn exam and any procedures done and this is included in the babies’ EMR.

· Creation of a record for each twin in the hospital ADT system results in the creation of a new record in the EHR for each
