Hannah Lambert, 16, has recently become withdrawn and depressed with lethargy, insomnia, worsening school performance. Her father and stepmother are very concerned and have her see their family physician, Dr. Black.
Dr. Black reviews the history of the mood changes with Hannah. She is not forthcoming about the recent abortion but is visibly upset and hints at recent medical treatment. Dr. Black sees in her record that there are masked encounters and discusses this with Hannah, who eventually reveals some of the history and gives him permission to view all of the information. Dr. Black diagnoses depression and refers Hannah to a private community psychologist.
Unfortunately, Hannah has irregular follow up with Dr. Black and the psychologist over the next 6 months.
Hannah’s mother moves to a rural area of another province and Hannah chooses to move with her. She eventually finds a family physician there, Dr. Schwartz who reviews her records. She happens to notice the abnormal Pap result and referral for colposcopy, but realizes there is no colposcopy report in the record. She asks Hannah about this, who reports she forgot to go to the appointment. Dr. Schwartz makes a referral to the local gynaecologist.
Hannah continues to have a markedly altered mood over time. As a result, Dr. Schwartz makes a referral for a telehealth consultation with a psychiatrist.
The telehealth consultation takes place three weeks later. The psychiatrist is located at the hospital in the nearest city and Hannah is at the telehealth unit in her local hospital. Following the consultation, Dr. Ho creates her consultation report and Dr. Schwartz is able to view a copy.
