Hannah Lambert’s mother moves to a rural area of another province and Hannah chooses to move with her. She is continuing to feel depressed and is wondering if an antidepressant medication might help. She makes an appointment with the local family doctor, Dr. Schwartz.
When she arrives at the clinic, Hannah stops first at the registration desk where the receptionist records demographic information about her and creates a record of her visit in the clinic’s ADT system. Hannah signs a consent form related to the clinic’s storage and use of her health information and the receptionist creates an EMR for her.
Dr. Schwartz talks with Hannah about her symptoms. Hannah feels very comfortable with her and relates the history. She gives Dr. Schwartz permission to unmask her records and view the content. Given the presenting problem of this new patient, Dr. Schwartz chooses to quickly view the most recent information and searches for the last five encounters with the health care system. She sees the clinic notes from Dr. Black and the psychologist and also happens to notice the abnormal Pap result. She is able to see the referral for colposcopy (done by Dr. Singh at the walk-in clinic) but realizes there is no colposcopy report in the record. She asks Hannah about this, who reports she did not feel up to going to the appointment, so did not attend. Dr. Schwartz realizes she must follow up on the Pap result, in addition to addressing the presenting complaint of depression.
Dr. Schwartz counsels Hannah about her depression and prescribes an antidepressant medication. She also makes a referral to a gynaecologist. Dr. Schwartz creates a record of the encounter in her EMR.

This health service follows upon the depression assessment health service (see 3.1.1) and describes all the health service events taken during the patient visit to a new family clinic in other jurisdiction where the patient moved.
Each health service event takes place during a same period of time (e.g. without cut of time), in presence or not of involved client; the health service events can be grouped in a health service encounter.
