Hannah Lambert, 17, has been diagnosed with depression and was recently started on an antidepressant medication. She lives in a rural area, far removed from a centre with psychological counselling. Recently, her mood has heightened markedly, with Hannah becoming very much more outgoing, gregarious and somewhat hyperactive. She has developed insomnia and shown some impulsive decision-making. Her mother is starting to become concerned about such a dramatic shift in Hannah’s behaviour and has taken her to see Dr. Schwartz again.
Dr. Schwartz talks with Hannah about the recent developments. Hannah feels she is doing very well, much better than before. She is extremely talkative and jumps quickly between ideas. Dr. Schwartz is concerned about the change and wonders if this is a manic episode and if Hannah’s diagnosis is actually bipolar affective disorder. She would very much like the input of a psychiatrist but there are none in her very remote rural area. Dr. Schwartz, therefore, makes a referral for a psychiatric Telehealth consultation. She completes her note of the encounter on her EMR and fills in a requisition for the consultation. She asks her clinic nurse to arrange for the appointment.
The nurse initially speaks with Hannah and her mother and provides them information about the Telehealth consultation. She sends the requisition to the intended psychiatrist’s office staff who respond later that day with available times. The nurse then contacts the Telehealth network coordinator to get a booking. The coordinator then notifies both Hannah and the psychiatrist, Dr. Ho.

This health service follows upon the new family physician encounter (see 3.1.2) and describes all the health service events taken during the patient visit, for the second time, to a new family clinic in other jurisdiction where the patient moved.
Each health service event takes place during a same period of time (e.g. without cut of time), in presence or not of involved client; the health service events can be grouped in a health service encounter.
