Mabel Lambert is diagnosed by her family doctor, Dr. Martin, as having Type II Diabetes mellitus. He counsels her about lifestyle changes, prescribes an oral hypoglycaemic medication and refers her to the diabetes clinic in the local hospital.
Four weeks later, Mabel is seen at the diabetes clinic. She is assessed by an internist and meets with a registered nurse, dietician and pharmacist. She is also seen by a podiatrist. All providers create a report of their recommendations that are sent to Dr. Martin.
Years pass and Mabel is fairly compliant with her diabetes management. An ACE inhibitor has been added to her medications within the last few months for hypertension. Mabel checks her sugars daily and uploads the results to Dr. Martin’s EMR using her home computer. She is able to do the same for her blood pressure. She can look at flow charts of her glucometer and blood pressure readings and of her HbA1C levels online.
Mabel is on an extended vacation with her husband Cyril in their motor home in a rural area of another province when she starts to feel poorly with malaise, increased thirst and frequent urination. Being on vacation, she has relaxed her attention to her diet and exercise. Along the way she has misplaced her glucometer and has not been able to check her sugars. She goes to the local ER there to be assessed. Unfortunately, she forgot her medications in the motor home and can’t remember what she is on. She does recall the blood pressure pill ran out a couple of days ago. The physician in the ER assesses her and finds both her blood pressure and blood sugar to be elevated.
Mabel is admitted to hospital for a couple of days until her sugar is under better control. On discharge, she and Cyril return home and the next day she sees Dr. Martin. Dr. Martin reviews her records and is able to see the details of her recent ER visit and hospitalization.
