1 Description of Health Service Encounter

DM Clinic (5.1.2) Storyboard: Diabetes (5.1)

Mabel Lambert was recently diagnosed with Type II Diabetes mellitus. As a part of her initial management, her family physician referred her to the local hospital’s outpatient diabetes clinic. Mabel presents to the clinic on the appointed day and is initially seen by an internal medicine specialist. Afterwards, she meets with a nutritionist.

The nutritionist reviews Mabel’s diet with her and searches her records for the most recent blood glucose results. Together they create a nutrition plan to help reduce Mabel’s weight and blood sugar. The nutritionist creates a record of the encounter in the hospital EMR.

Mabel goes on to see the diabetes nurse and a podiatrist that day. Each creates a record of the encounter in the EMR.

Assumptions

· Able to search EHR based on segment of data (lab results) and specific type of test

· All providers use hospital EMR for keeping clinical records of outpatient encounter

This health service follows upon the referral to DM clinic (see 5.1.1) and describes all the health service events taken during a visit to local hospital’s outpatient diabetes clinic and, more specifically, during and encounter with the nutritionist.

Each health service event takes place during a same period of time (e.g. without cut of time), in presence or not of involved client; the health service events can be grouped in a health service encounter.
