Description of Health Service Encounter

Mabel Lambert was diagnosed with Type II Diabetes mellitus 5 years prior. She is assessed twice a year at the diabetes clinic in the local hospital and is followed regularly by her family physician, Dr. Martin. In addition to dietary and lifestyle changes, current management includes an oral hypoglycaemic agent. Unfortunately, Mabel is not very compliant with her diet and exercise but she does check her sugars and blood pressure daily at home. About one year ago, Mabel was diagnosed with hypertension and was started on an ACE inhibitor.

Once a day, Mabel uses her glucometer to check her blood sugar and her digital sphygmomanometer to check her blood pressure. She has come to enjoy using the technology to monitor her conditions. She is able to connect her glucometer and cuff to her computer and automatically upload the values to an application provided by Dr. Martin’s office. With this application, copies of the data are sent to her doctor’s EMR. At her convenience, Mabel is able to view her results for the last month, for example, and view them graphically. She finds this very helpful to follow her progress in treating both conditions. Dr. Martin and the diabetes clinic staff are able to periodically review Mabel’s results as well.

This health service follows upon the DM clinic visit (see 5.1.2) and describes all the health service events taken during the patient home access health service; the patient lived in the involved jurisdiction.

Each health service event takes place during a same period of time (e.g. without cut of time), in presence or not of involved client; the health service events can be grouped in a health service encounter.
