Description of Health Service Encounter

Cyril Lambert was diagnosed with atrial fibrillation about six months ago. He has been very non-compliant with taking his Coumadin. Four hours ago he started to develop some weakness in the left side of his body and face. He chose to ignore it but it has progressed and Cyril is starting to become concerned. When Cyril’s wife, Mabel, came home from work she fears he is having a stroke and calls an ambulance.

The ambulance is en route to Cyril and Mabel’s home, one town over from the regional hospital. One of the emergency medical technicians (EMT) in the ambulance has patched into the hospital’s information system and has been able to review information through Cyril’s EMR there. He retrieves Cyril’s health profile and reviews his complete past medical history, current problem list, allergies and medications.

The EMTs transport Cyril to the regional hospital’s emergency department.

Assumptions

· EMTs have access to EHR

· EHR accessible remotely (on the road)

· EMTs have permission to access a person’s EHR before arriving at that person

Health Service

This health service describes all the health service events taken during the patient’s transport by emergency medical technicians (EMT), in the jurisdiction where the patient lives.

The patient also obtained other health services encounters for a chronic obstructive pulmonary disease (COPD – see 1.1.1); at this period, a health service encounter took place in another jurisdiction.

Each health service event takes place during a same period of time (e.g. without cut of time), in presence or not of involved client; the health service events can be grouped in a health service encounter.
