Home care (7.1.4) Storyboard: CVA (7.1)
Cyril Lambert recently suffered a stroke and was hospitalized initially. He has been discharged home and assessed through the local Community Care Access Centre. The case manager there has arranged for a number of home services, including physiotherapy.
The physiotherapist assigned to work with Cyril accesses his records and searches for his last 5 encounters. She drills down into the recent hospitalization encounter and reads the discharge summary and physiotherapy notes . From this she has a good impression of the history of Cyril’s stroke, his deficits from it, the treatment thus far and the ongoing management plan.
The physiotherapist assesses Cyril in his home. She performs her evaluation and commences treatment with him. At the end of the session she creates a record of the encounter in her CCAC information system.
Assumptions
· All of the hospitalization records are contained within a single hospital encounter in the EHR and one can drill down into this to view details
· The physiotherapist has consent to view the discharge summary and certain other hospital encounter data
· The physiotherapist has an CCAC Information system

This health service follows upon the CCAC visit (see 7.1.3) and describes all the health service events associates with home services (including physiotherapy) given by the local Community Care Access Centre after that the patient has been discharged home and assessed through this centre.
The patient also obtained other health services encounters for a chronic obstructive pulmonary disease (COPD – see 1.1.1); at this period, a health service encounter took place in another jurisdiction.
Each health service event takes place during a same period of time (e.g. without cut of time), in presence or not of involved client; the health service events can be grouped in a health service encounter.
