Description of Health Service Encounter

Dr. Black saw Gareth Lambert, 6, for a sore throat on Wednesday, three days prior. Dr. Black shares call with physicians from another clinic. Dr. White, from the other clinic, is on call on Saturday when she gets a message from Linda Lambert. Gareth is no better, but no worse, and she is wondering about the results of the throat swab.

Through the two clinics’ call arrangement, Dr. White has permission to view Gareth’s records. She accesses them through her computer at home and searches for the last five encounters with the health care system. From what is returned, she sees the information related to Gareth’s visit to Dr. Black three days prior and that the throat swab has been processed and the result was made available late last evening. The swab is positive for Group A Streptococcus. Dr. White reviews Gareth’s health profile and sees that he has no recorded drug allergies.

Dr. White calls Linda Lambert and reports the results. She reviews with her the information in Gareth’s health profile; particularly, that he has no history of allergy to penicillin. Linda indicates that Gareth did, in fact, react to Amoxil nine months prior when he had otitis media. Dr. White searches all encounters in the last 12 months and can find no record of this reaction or ear infection. Linda asks to confirm Dr. White is looking at Gareth’s chart and not that of his twin brother, Tim. Dr. White accesses Tim’s records and sees that the otitis media and Amoxil reaction were recorded there, for the wrong brother.

Dr. White e-prescribes an appropriate antibiotic for Gareth. She creates a chart for him in her clinic’s EMR and makes a record of the encounter. She chooses to save a copy of the throat swab report for inclusion in her clinic’s EMR for Gareth. Dr. White emails a message to Dr. Black notifying him of the error in the records.
