Description of Health Service Encounter

(Author: Brent Jones – April 12th, 2005)

Pharmacy (11.1.2) Storyboard: Strep Throat (11.1)

Gareth Lambert has been diagnosed with strep throat. The physician on call that weekend for Dr. Black’s office has e-prescribed an antibiotic. Gareth’s mother, Linda, has gone to the pharmacy to pick up the medication.

Linda informs the pharmacist she is there to obtain the prescription for Gareth and that she has used that pharmacy for him in the past. The pharmacist reviews Gareth’s demographic information with Linda and she realizes she has forgotten to inform the pharmacy (and others) of their recent move within the city. Linda provides their new address which the pharmacist records in the pharmacy’s information system. He obtains Gareth’s prescription for the antibiotic and dispenses it. The pharmacist makes a record of the encounter.
