Strep Throat: Storyboard
Linda Lambert takes her son Gareth, 5, to their family physician,
Dr. Black. Gareth has been unwell for a couple of days and is now
complaining of left ear pain. Dr. Black examines him and diagnoses
otitis media and e-prescribes a course of amoxicillin. He creates
an encounter note in the clinic’s EMR.
Linda obtains the prescription from the pharmacy and starts Gareth
on it, but he soon develops an itchy rash over his body. Linda
takes him back to see Dr. Black who diagnoses an allergy to
penicillin. The Amoxil is stopped and another antibiotic is
e-prescribed. Dr. Black again creates an encounter note in the
EMR.
Nine months later, Linda Lambert takes Gareth back to see Dr.
Black. Gareth complains of a sore throat and malaise. Dr. Black
examines Gareth and sees non-specific findings that are most
suggestive of an early viral upper respiratory tract infection. He
does a throat swab and sends it to the lab. He suggests symptomatic
treatment and records his findings in an encounter note in the
EMR.
Dr. Black shares call with physicians from another clinic. Dr.
White, from the other clinic, is on call on Saturday when she gets
a call from Linda Lambert. Gareth is no better, but no worse, and
she is wondering about the results of the throat swab. Dr. White is
able to view Gareth’s information and retrieve the test result.
While doing this she detects an error in his record. Dr. White
e-prescribes an appropriate antibiotic for Gareth and then e-mails
a note to alert Dr. Black’s office of the error.
Linda goes to the pharmacy to obtain the medication for Gareth.
While there, she remembers to give the pharmacist the Lambert’ new
address, as they moved very recently.
