The physician does a request (with criteria) through her clinic’s EMR system to see all encounters in the last twelve months. The physician looks at the summary list of health service encounters and can find no record of this reaction or ear infection. The patient’s mother asks to confirm physician is looking at patient’s chart and not that of her twin brother.
